A COUPLE OF DAYS AFTER PATIENT GOES HOME FROM BIRTH 2 WEEKS OF AGE

2 MONTHS OF AGE: DTaP, IPV, Hep B (Pediarix), Pneumococcal (Prevnar 13), HIB (PedVax), Rotavirus (Rotarix)

4 MONTHS OF AGE: DTaP, IPV, Hep B (Pediarix), Pneumococcal (Prevnar 13), HIB (PedVax), Rotavirus (Rotarix)

6 MONTHS OF AGE: (Must be at least 6mo. Old for this apt. and at least 8 weeks from their 4 mo. well ck) DTaP, IPV, Hep B (Pediarix), must be 8 weeks b/t dose #2 and Dose #3 of Hep B, Pneumococcal (Prevnar 13), Influenza #1(Dose #2 in 4 weeks)

9 MONTHS OF AGE: TB (Fill out TB Questionaire), Hematocrit

12 MONTHS OF AGE: Varicella (Varivax), MMR, (MMRV if available), Pneumococcal (Prevnar 13) (OLD SCHED.) Varicella (Varivax), MMR, (MMRV if available), Pneumococcal (Prevnar 13) Hep A (NEW SCHED.)

15 MONTHS OF AGE: DTaP, HIB, Hep A #1 (OLD SCHED.) DTaP, HIB (NEW SCHED. If they had Hep A. @ 12 months.)

18 MONTHS OF AGE: HEP A #2 (NEW SCHED.), MCHAT (Make sure there is 6 mo. From 1^st Hep A.)

24 MONTHS OF AGE: Hep. A #2 (OLD SCHED.)

3 YEARS OF AGE: Update any vaccine deficits

4 YEARS OF AGE: , (Must be 4 years of age), DTaP, IPV = Kinrix, MMR, Chicken Pox (Varicella), (MMRV if available)

5 YEARS OF AGE: Update any vaccine deficits

6 YEARS OF AGE: Update any vaccine deficits

8 YEARS OF AGE: Update any vaccine deficits

10 YEARS OF AGE: Tdap, HPV (Cervarix for females and Gardasil for males), update any vaccine deficits

12 YEARS OF AGE: Tdap, Meningococcal (Menactra), HPV for females and males

14 YEARS OF AGE: Update any vaccine deficits

16 YEARS OF AGE: Menactra booster (3-5 years after 1^st dose) Update any vaccine deficits

18 YEARS OF AGE: Update any vaccine deficits

DIRECTIONS TO 5301 RENO CORPORATE DRIVE

The new office is a little difficult to find but we hope that the following directions will help you get to us quickly. We are located off Longley Lane and Barron Way.

Off Longley Lane turn East onto Barron Way, take the second RIGHT which is a private drive, there are no street signs on this private drive. Take the first LEFT and you are entering our parking lot. Turn LEFT again, right after the building that is on your left. You will see cars parked in front of our building which is #5301. The building is a chocolate brown color with thin white accent stripes. We are physically loacted on the corner of Barron Way and the private drive with no name. If you have any difficulty finding us, please don’t hesitate to call our office.
